Mullis and Associates Physical Therapy currently accepts the following health insurances. If you do not see your insurance on the right, please check back regularly as we are continually adding new plans.
Mullis PT recommends that you contact your insurance company, before starting physical therapy, to make sure that you fully understand your physical therapy benefit.
Please obtain a referral from your Primary Care Physician (PCP) or a specialist before scheduling your appointment.
All co-payments will be collected at the time of service.
